Pro Fitness Membership Payment Authorization Agreement
By completing and signing this form, you, the undersigned member, hereby authorize Pro Fitness (the "Company") to process payments for your membership and any related fees using the updated payment card details provided. This authorization applies to both future scheduled payments and any past-due amounts owed under your membership agreement.
1. Authorization for Payment
You authorize Pro Fitness to:
Debit your updated payment card for recurring membership fees and any additional services as outlined in your membership agreement.
Charge your updated payment card for any outstanding payments that were not successfully processed due to invalid or expired card information.
2. Validity of Authorization
This authorization remains in effect until:
Your membership is canceled in accordance with the cancellation terms of your membership agreement; or
You provide Pro Fitness with updated card information or written notice revoking this authorization.
3. Member Responsibilities
You agree to:
Ensure that the payment card provided remains valid and has sufficient funds or credit available to cover your scheduled payments.
Notify Pro Fitness immediately if your card is lost, stolen, or replaced.
4. Rights and Acknowledgments
You acknowledge and agree that:
Pro Fitness reserves the right to assess fees as outlined in your membership agreement for failed payment attempts or late payments.
Pro Fitness may share your payment information securely with its payment processing partners solely for the purpose of facilitating payments under this authorization.
It is your responsibility to review your membership account and notify Pro Fitness of any discrepancies in charges.
5. Consent for Payment Processing
By signing below, you confirm that:
You are the authorized user of the payment card listed on this form.
You agree to the terms outlined above and authorize Pro Fitness to process payments as described.
You understand and accept that this authorization applies to both future payments and any past-due amounts as permitted by your membership agreement.
